Acute Stroke Care Coordination

EMS transports Mary for a possible stroke, notifying the hospital en route and preregistering her, making consent forms available upon their arrival. EMS is also able to capture mobile imaging of Mary’s facial droop to display in the EHR via the health information exchange. The ED physician can also access Mary’s historical information during the encounter. Following initial treatment, she is referred to a neurologist, and pre-registers electronically. Faxed documents are converted and sent through direct messaging from the PCP to the neurologist. Pre-hospital and hospital data are appropriately merged into the stroke registry to track and improve time to treatment. Mary is referred for rehabilitation services with all medical information available electronically.

Dawn's daughter notices her mother has become increasingly aggressive, wanders away from home and occasionally gets lost. In a visit with Dawn's longtime PCP, the PCP suggests Dawn be referred to a neurologist for a dementia evaluation. The neurologist evaluates Dawn and establishes that she has multi-infarct dementia with behavioral disturbance, requiring care in a long-term skilled nursing facility (SNF). With Dawn's PCP and daughter, they arrange for her transfer to the SNF.
